3. Preferred provider organizations (PPOs). PPOs have contracts with certain health care providers. These policies provide a list of the health care providers that they endorse or contract with, otherwise known as in-network providers and you get the greatest benefit when you seek treatment from in-network providers. Seeking treatment from a health care provider out-of-network is more costly and generally has lesser benefits.

4. Self-insured health plans. Self-insured health plans are often seen among much larger or national companies such as a municipality or a school district, or members of a workers’ union. Members of such groups often contribute at least a part of the cost of the health care plan, and these plans are managed by a third party administrator or TPA.
